“Describe a time you found a defect after an initial successful test.”

In a personal inventory application, the total looked correct for a small sample but changed unexpectedly when I imported a second file. I traced the issue to duplicate item identifiers and wrote a minimal example that reproduced it. I added validation and a clear duplicate-handling rule, then checked both repeated imports and new records. The test data produced the expected totals afterwards. I documented the rule so another contributor could understand why duplicates were rejected. I would not describe this as a production savings claim because it was a project. It shows how I turn an unclear result into a reproducible case and verify the resulting change.
